Your participation in this study is completely voluntary. This means that you do not have tovparticipate in this study unless you want to. You may also withdraw from participating in the study at any time.

About the Survey

This study aims to determine if Double Up participants increase fruit and vegetable consumption after participation in a Nutrition Incentive Program. The survey will consist of questions about your fruit and vegetable consumption behaviors. Completing the survey will take roughly 20 minutes of your time. No personal identifying questions will be asked, and all responses will be kept strictly confidential.

About the Gift Card

Participants may receive a $10 digital gift card sent to them via email, or if they do not have an email address, they may receive it through standard mail.

At the end of the survey, participants will be prompted to provide an email or mailing address. SFC staff will then issue the digital gift card within 2-3 days!
